約1200名が参加した臨床試験で、89%入院、死亡を減らしたというのは治療薬としては驚くような効果だと思います。メルクで50%、抗体製剤で70%という数字が示されていたのと見比べても、とても高い効果であることが見てとれます。ただし、これは必ずしも「メルクや抗体製剤よりも有効である」ということを示すものではありません。対象となった人や投与のタイミングなども異なるので、異なる試験から生まれた結果の単純比較というのはできません。この試験では、メルクと同様、60歳以上の高齢者または重症化リスクのある人が対象になっており、今後承認された場合には、適応となる人も同様の人になる見込みです。また、ワクチン未接種の方のみを含めています。ワクチン接種後の方はそもそも重症化リスクが遥かに低下するので、薬の効果への期待値は下がり、その評価もしにくくなるからです。これらの方がCOVID発症から3日以内に5日間薬を飲むグループと偽の薬を飲むグループに分けられ、その後のCOVIDによる入院や死亡の数が観察され、89%減少という高い効果を確認しています。用いられた薬は、プロテアーゼ阻害剤と呼ばれるタイプの薬（正確には2種類の薬からなる）で、これまでHIVやC型肝炎ウイルスの治療薬として同種の薬が用いられた実績があります。メルクの薬が、ウイルスの変異を促してウイルスを減らすというタイプの薬で、この作用が新たな変異ウイルスの脅威を生むのではないかという懸念が上がっているのに対して、当該薬剤にその懸念はないと言えます。ただし、1000人単位の比較的小さな試験での確認にとどまるので、稀な副作用の観察はできておらず、今後汎用された際に稀な副作用がどの程度生じるのかには注意が必要です。一般に薬剤の副作用のリスクというのはワクチン以上に高くなりますので、重篤な副作用が出ないかについては慎重に観察される必要があります。また、5日間で合計30錠、70000円程度が見込まれている薬剤ですので、多くの方が内服をスケジュール通りにきっちり守れるのかという点や、（ディスカウントは計画されていますが）発展途上国と先進国の格差を広げる可能性なども懸念材料として挙げられます。
